Title:Home Care Interventionist - Bilingual
The Health Promotion Interventionist is instrumental in providing support to Home Assist Health Clients as directed by the Health Promotion Practitioner. Engaging clients in healthier living patterns through proven 1:1 teaching and behavior modification strategies. The Interventionist follows an intervention plan to conduct home visits and telephone calls to assist in care coordination and provide coaching to clients, family members and caregivers. Education and skill building are designed to attain, keep or improve skills for effective self-care. The Interventionist’s efforts are designed to empower clients and address barriers to health. A successful Interventionist is adaptable and comfortable working within evolving systems, can take initiative and concept ideas for practice improvement, is a self-starter and team player, and performs their work with cultural humility.

Essential Functions
  • Performs as a representative of Home Assist Health upholding the company mission, vision, and purpose.
  • Follow individualized intervention plans based on clients’ needs, situations, strengths, and support networks to achieve intended goals.
  • Work collaboratively internally, externally and with clinical teams to engage, educate, communicate, and coordinate care. Ensure that all services prescribed in the individualized service plan are implemented.
  • Help people understand their health condition(s) and implement strategies to improve their health and wellbeing.
  • Coach patients in effectively managing their chronic health conditions and self-care.
  • Conduct home care visits to support client’s transitioning from hospital to home, to achieve compliance with medications, appointments and other instructed activities.
  • Connect Home Assist Health clients to community-based resources to directly address social determinates of health. This could include but is not limited to delivering food boxes, supporting a long-term care application or utility assistance
  • Perform duties telephonically, in-person, in-home and in the community.
  • Document activities, intervention reports, and results in an effective manner while strictly adhering to the policies and procedures in place

Additional Functions

  • Provide client services as necessary whether within a home visit or to cover a shift to prevent gap in services.
  • Assist with special projects.
  • Other duties as assigned

  • High School Diploma or equivalent required
Skills & Abilities
  • Requires the ability to read, write and speak effectively in English.
  • Requires the ability to read, write and speak effectively in Spanish, native proficiency preferred.
  • Must be proficient in computer skills including Microsoft Outlook, Word and Excel.
  • Must possess exceptional customer service skills.
  • Must possess good communication skills both verbal and written
  • Must be able to understand and follow directions, policies and procedures.
  • Must have strong self-management skills.
  • Must be at least 18 years of age.
  • Current CPR and First Aid
  • AHCCCS Direct Care Worker Training or current AZBN CNA/LNA
  • AHCCCS Direct Care Worker 2-DDD Training
  • Current Article 9
  • Skill Building and ISP Process or Habilitation training
  • Must have a valid driver’s license in good standing and maintain current vehicle insurance and registration.
  • Required: Direct patient contact experience
  •  Preferred: Community health worker, Medical Assistant, Habilitation aide, CNA
Background Requirements
  • Must have DPS Level 1 Fingerprint Clearance (or be able to obtain one within 90 days hire).
  • Must pass Criminal Background if applying for DPS Level 1 Fingerprint Clearance Card.
  • Must pass APS Registry Check at new hire and annually.
  • Must sign notarized Criminal History Affidavit at new hire and every 3 years.

Immunization Requirements

  • Must have proof of TB Clearance
