Description

At Home Assist Health, the Health Promotion Practitioner I is instrumental in providing support to Health Promotion clients as directed by the Health Promotion Practitioner II by engaging them in healthier living patterns through telephonic care coordination and proven 1:1 teaching and behavior modification strategies. The Practitioner I monitors intervention plan progress, makes updates and revisions as well as reports on outcomes as directed by Practitioner II. The Practitioner I works closely with the Interventionist on implementation of intervention plan strategies and 1:1 teaching and behavior modification strategies.

 

Job Functions

 

Essential Functions

  • Acts as a positive representative of Home Assist Health upholding the company’s Mission, Vision and Purpose.
  • Performs initial phone intake including risk assessments which may include:
    • Hospital Readmission Risk Questionnaire
    • Fall Risk Assessment
    • Social Determinants of Health Assessment
  • Provides in-depth phone screenings addressing discharge plans, medication review, appointment adherence and social determinates of health.
  • Triage phone calls, referrals and conduct home care visits to support client’s transitioning from hospital to home, to achieve compliance with medications, appointments and other instructed activities.
  • Relays pertinent information to the Practitioner and Provider in a timely manner.
  • Assists in the management of patient utilization of the designed health care system, engages patient in community resources and provides patient education
  • Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with recipient, their family, behavioral and general medical and dental health care providers, community resources and others to ensure that all services prescribed in the individualized service plan are implemented
  • Help people understand their health condition(s) and develop strategies to improve their health and well being
  • Coach patients in effective management of their chronic health conditions and self-care
  • Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place
  • Build and maintain positive working relationships with the clients, providers, case managers, supervisors and office staff
  • Advocate for and help clients get resources that would improve their well-being
  • Assist with other duties as assigned.

 

Qualifications

 

Education

  • High School Diploma or equivalent plus vocational training or healthcare specific college experience (i.e. MA, CNA, LPN, etc.)

 

Skills & Abilities

  • Requires the ability to read, write and speak effectively in English.
  • Requires the ability to read, write and speak effectively in Spanish, native proficiency preferred.
  • Requires ability to professionally communicate with other Health Professionals at all levels
  • Must be proficient in computer skills including Microsoft Outlook, Word and Excel.
  • Requires proficiency in time management, organizational skills, interviewing, and relationship-building
  • Must possess exceptional customer service skills.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.

Training/Certification/Licensure

  • AHCCCS Direct Care Worker Training or current AZBN RN/LPN/CNA/LNA.
  • DCW Developmental Disability training module.
  • Skill Building/ISP Process or Habilitation Training.
  • Article 9 Training.
  • Prevention & Support Training required for specific services.
  • Current CPR and First Aid Certifications required

Experience

  • Required: Direct patient contact experience
  • Preferred: Community health worker, Medical Assistant, Habilitation aide, CNA

Background Requirements

  • Must have DPS Level 1 Fingerprint Clearance (or be able to obtain one within 90 days hire).
  • Must pass Criminal Background if applying for DPS Level 1 Fingerprint Clearance Card
  • Must pass APS Registry Check at new hire and annually
  • Must sign notarized Criminal History Affidavit at new hire and annually

Other

  • Requires reliable transportation and current driver’s license
  • Must always maintain current automobile registration and automobile insurance
  • Must provide a negative tuberculosis (TB) test result on an annual basis
